Welcome! I am looking forward to meeting all the new and returning students and their families. After enrollment I will be letting parents know of any missing items that I need for each student’s file such as physicals, shot records, allergy or asthma forms. Please get these items to me as soon as possible. We also need home, work, and emergency contact numbers to add to each child’s record in case of an emergency. Prescription medication will need to be in its original container with a proper prescription label and appropriate paperwork filled/signed out by both the physician and parent. For those students that had prescription medication last year, they will need a new prescription medication with current prescription label and new signed paperwork by physician and parent. Note: this form is available on the Bradley website under the “Nurse” tab and “Health Forms” section. Kansas Law – Kansas requires all kindergarten students and any new students (new to Kansas), 8 years old and under, to have a school physical dated no later than January 1st 2011. Note that this is a onetime requirement. All students must have an up-to-date immunization record on file in the nurse’s office. New 2011 immunization changes are listed below. Three doses of Hepatitis B are required through 11th grade. Two doses of Varicella (chickenpox) are required for Kindergarten, 1st, 2nd and 7th grade; one dose is required for grades 3-6 and 8-11 unless proof of disease documented by a licensed physician is provided (note that two doses are recommended for all ages). Booster dose of Tdap (Tetanus and Pertussis) is required at 7-9th grade if no previous history of Tdap vaccination regardless of interval since the last dose of Td (Tetanus). Two doses of Hepatitis A are required for children less than 5 years of age in our early childhood (Pre-Kindergarten) program. One dose of Meningitis (MCV4) is recommended at age 11 with a booster dose at 16 years of age.
